Cholesterol and heart risk: why the total number is not enough

The body needs cholesterol, but elevated low-density lipoprotein, or LDL, cholesterol contributes to atherosclerosis. Treatment decisions should consider overall cardiovascular risk, rather than the total cholesterol number alone.

What does a lipid profile show?

A lipid profile usually measures total cholesterol, LDL, HDL and triglycerides. LDL is a major factor in atherosclerosis and the main target of lipid-lowering treatment. HDL helps transport cholesterol, but a very high HDL does not automatically cancel out raised LDL risk. Triglycerides are affected by metabolism, alcohol, diet and other factors. Why is one person’s target stricter than another’s?

The desired LDL level depends on previous cardiovascular disease, diabetes, kidney disease, very high blood pressure and other risk factors. The doctor assesses risk using age, sex, smoking, blood pressure and laboratory data. The same LDL result can therefore lead to different recommendations for two people. What can lifestyle changes achieve?

Regular activity, not smoking, weight reduction where needed and a diet with more vegetables, pulses, whole grains, fish and unsaturated fats improve overall risk. Reduce trans fats and excessive saturated fat. Lifestyle is the foundation, but in high-risk cases it does not replace prescribed treatment.

Will medication be lifelong?

For many people, lipid-lowering treatment reduces long-term risk. Decisions should be made with a doctor, considering expected benefits, possible side effects and patient priorities.
